Vendor Information Sheet - must be signed by President/CEO or Owner
a. Read EEI Vendor Code of Conduct V1 2023
b. Consent must be signed by President, CEO, Owner, Propietor or Officer only
c. “No acknowledgement Consent Form” signed by vendor, NO AWARDING OF PROJECT
